Enrollment Procedures:
The following forms are required by the state of Florida and need to be completed prior to placement.

 

1. Florida Department of Children and Families Application
2. Complete and up to date record of immunizations.
3. Complete and up to date Health examine
4. Signed Contract.
Note: All forms must be updated at least every year or sooner according to need.
